Engage Patients and Families in Person-Centered Co-Design

A critical component of improving care is recognizing that each individual patient is the true expert on their own health. While your initial intervention may seem tailored to the individual's population segment, every community is different and every program is best when tailored to the lived experience of those it is trying to help. Many people with complex needs have experienced disrespectful treatment in health care settings. Like health care providers, people with complex needs are also frustrated by the juxtaposition of high costs and utilization paired with poor health outcomes.
