Develop the Necessary Partnerships

Many people with complex needs have care needs that are well outside of the bounds of traditional health care or usual health system structures. Funding and organizational models can put critical needs in different silos, whether for behavioral / mental health, dental health, home care, long-term care, or for housing, transportation, food security, social supports or other “social determinant” needs.  Enhanced care models must have a plan in place to address unmet needs, no matter how these services are funded, in order to see and sustain improvement. Cultivating partnerships with clinical, social service and community-based organizations to meet these needs is key to providing better health care for people with complex needs.
